La mesure du rapport MT, diamètre maximum de la masse médiastinale/diamètre transverse du thorax en T6-7, pour définir une atteinte médiastinale volumineuse dans le lymphome de Hodgkin.

CT-scan
Différentes modalités ont été recommandées pour définir une atteinte volumineuse.
Masse médiastinale, mesurée dans le plan axial, puis la dimension maximum quel que soit le plan de mesure.
Autres localisations ganglionnaires et extra-ganglionnaires mesurées selon leur dimension maximum. L’atteinte volumineuse est définie selon différents seuils entre 5 et 10 cm.

TEP-FDG
Masse volumineuse principale, mesurée selon les mêmes critères que sur le CT-scan.

Lymphome de Hodgkin
Selon la classification d’Ann Arbor – Cotswolds une masse ganglionnaire unique ≥ 10 cm ou une masse médiastinale de diamètre égal ou supérieur au tiers du diamètre transverse intra-thoracique, avec une valeur différente selon les groupes coopérateurs.

EORTC/LYSA : rapport MT ≥ 0.35
GHSG : rapport MT ≥ 0.33

Masse ganglionnaire ou extra-ganglionnaire ≥ 10 cm de plus en plus utilisée dans les essais cliniques.

Lymphome B diffus à grandes cellules
Masse ganglionnaire ou extra-ganglionnaire ≥ 10 cm

Lymphome folliculaire
Selon les critères du GELF, masse ganglionnaire ou extra-ganglionnaire > 7 cm dans son plus grand diamètre.

Une atteinte volumineuse est un facteur de risque et influence le choix du traitement initial.

Les critères d’atteinte volumineuse varient selon les essais cliniques récents.

Un logiciel permettant sur un CT-scan une mesure tridimensionnelle d’une masse ganglionnaire ou extra-ganglionnaire n’a pas été développé. Le volume tumoral métabolique total (TMTV) obtenu en additionnant le volume métabolique des lésions ganglionnaires et extra-ganglionnaires, est évalué dans les essais cliniques récents. Cependant les méthodes de mesures du TMTV ne sont pas encore standardisées pour les principaux lymphomes avides de glucose. La valeur prédictive du TMTV pour la survie sans progression est confirmée pour plusieurs lymphomes, mais il ne remplace pas encore dans les index pronostiques, l’atteinte volumineuse définie selon les critères classiques.

La masse ganglionnaire ou extra-ganglionnaire la plus volumineuse est mesurée dans sa dimension maximum sur le CT scanner ou pour les lymphomes avides de glucose, sur la TEP-FDG.
