Traitements standards

Le traitement du lymphome de Hodgkin (LH) en 1ère ligne est adapté à la fois aux facteurs de risque (FR) et
à la réponse précoce à la chimiothérapie. Le stade d’extension et les facteurs de risque définissent des
groupes de risque, selon les critères de l’EORTC (European Organisation for Research and Treatment of
Cancer) et du groupe allemand GHSG (German Hodgkin Study Group) qui correspondent à un consensus
européen. Les stades IIB à haut risque (atteinte médiastinale volumineuse et/ou extra-ganglionnaire de
contiguïté) sont traités comme les stades III-IV par chimiothérapie exclusive, par de nombreuses équipes.

La tomographie par émission de positons (TEP) réalisée après 2 cycles (TEP2) est prédictive, décisionnelle et
indispensable, quel que soit le stade initial du LH. L’échelle qualitative en 5 points de Deauville, score de
Deauville (SD), est le standard de l’évaluation de la réponse au traitement. La TEP2 permet d’optimiser le
rapport bénéfice-risque en modulant le traitement selon l’obtention ou non d’une réponse métabolique
complète, réduction ou renforcement de l’intensité de la chimiothérapie, et de définir les indications de la
radiothérapie.

Selon les facteurs de risque utilisés et la stratégie TEP-guidée, plusieurs traitements de référence sont
possibles pour un même groupe de risque. Pour les 3 principaux groupes de risque des patients adultes
âgés de moins de 60 ans, les traitements de référence sont présentés selon les critères de l’EORTC, du
GHSG, les autres groupes coopérateurs européens, et certaines recommandations nord-américaines, bien
que ces dernières s’éloignent parfois des traitements pratiqués en Europe.

Dans les cas où le BEACOPPesc n’est pas utilisable en 1ère intention, après discussion en RCP, un traitement par ABVD reste possible, mais en cas de TEP2 positive, le passage au BEACOPPesc se posera. Selon l’essai RATHL, le traitement guidé par la TEP après 2 cycles d’ABVD comporte, si TEP2 négative (SD 1, 2, 3) 4 cycles d’AVD (arrêt de la Bléomycine pour éviter une possible toxicité pulmonaire), et si TEP2 positive (SD 4, 5) 4 cycles de BEACOPPesc, sous réserve de TEP négative après 3 cycles de BEACOPPesc.
